The ERA test involves a mock cycle in which the endometrial lining is prepared exactly as it would be for a frozen embryo transfer, and a biopsy is taken at the standard transfer window. The biopsy is analyzed to determine whether the endometrium is receptive, pre-receptive, or post-receptive at that point. Studies suggest that 20–30% of patients who fail repeated transfers have a displaced implantation window. For those patients, adjusting the transfer timing to match their personal receptivity window can significantly improve outcomes.

If prior cycles did not include PGT-A, testing embryos from the next retrieval cycle is a meaningful change for patients with multiple failures. For patients who already used PGT-A and transferred euploid embryos without success, the workup shifts toward uterine and implantation factors rather than embryo quality.

Testing should be considered if prior cycles showed low fertilization, poor embryo quality, or high rates of embryo arrest. Single embryo transfer (eSET). Transferring more than one embryo does not meaningfully improve the odds of a live birth and substantially increases the risk of multiples.
